As a member of Community Care Associates, Inc. (CCA) you have certain health care rights and responsibilities.
Please familiarize yourself with the following.
Your rights include:
- Being treated with courtesy, dignity, and respect by member services
- Access to a list of Community Care Associate providers
- Choosing a Primary Care Physician (PCP) from our network
- The option of changing your Primary Physician
- Participating with your doctor regarding your health care
- Maintaining the privacy of your Protected Health Information subject to HIPPA regulations/guidelines
- Access to HealthChoice Plan regulations & guidelines
- Access to information about how to use CCA’s services
- CCA's Compliance with HealthChoice Plan regulations
Your responsibilities include:
- Notifying your employer of address/telephone changes
- Presenting your I.D. card to participating providers before treatment
- Immediately reporting loss/theft of your Identification Card
- Following HealthChoice Plan rules and regulations
- Following CCA’s rules for receiving health care services
- Assuring that no one else uses your card
- Choosing a Primary Care Physician (PCP)
- Treating Community Care Associates staff members with respect
- Knowing what to do if Community Care Associates offices are closed
- Accurately informing your PCP of your entire medical history
- Completing required forms for treatment
- Keeping your doctor’s appointments
- Reporting any suspected fraud
- Obtaining necessary pre-authorizations
